Master Site Plan

Project Updates

In order to best serve our residents, Evergreen has initiated a Master Site Plan project with six phases of development spanning five years. Learn about the Evergreen Foundation Capital Campaign which provides support for many of the projects.

Phase One

Phase One of the Master Site Plan project was completed in 2013 with the opening of the Creekview Rehabilitation Center at Evergreen and the relocation of rehab residents to the renovated Creekview Households 5 and 6, conveniently closer to Therapy Services. Included in the renovation was converting all rooms to private and adding full baths with showers to each room.
PHASE ONE COMPLETED.

Phase Two

Phase Two, the relocation of assisted living residents to the all-new Manor View in spring of 2014, put all 40 beds of the Garden Heights/Manor View Community Based Residential Facility on the same floor. Also included in this phase was an update to the Garden Heights common spaces and the conversion of the adjacent and formerly enclosed patio into an open sitting and garden area. Now residents have direct access and can conveniently enjoy the outdoors in a garden setting.
PHASE TWO COMPLETED.

Phase Three

Phase Three, the renovation of our memory care building ShareHaven, was completed in summer 2014. Private bathrooms were added to each of the 20 private ShareHaven accommodations and public spaces were expanded with the addition of an exercise room and a large four-seasons room.
PHASE THREE COMPLETED.

Phase Four

Phase Four began in early August and was completed mid-December 2014. Garden Terrace was converted into a Residential Care Apartment Complex like Garden Place with ten new assisted living apartments for those requiring the least amount of daily assistance.
PHASE FOUR COMPLETED.

Phase Five

Phase Five began in the spring of 2015 and will continue through 2017. The lower level Commons was expanded to create options for multiple meeting spaces. Included are improved sound technology with a hearing loop, four new screens and projectors, as well as updated lighting. New more accessible bathrooms have also been added. This lower level part of Phase Five was completed in late summer, 2015. Other plans in this Phase include changes to the appearance of the main building from Westfield Street. The façade of the main entrance and building front will be updated including the repair or replacement of windows in the Manor North and South Apartments.
PHASE FIVE IN PROGRESS.

Phase Six

If all goes as planned, Phase Six will begin in 2016 or 2017. Plans are to expand the Manor Building to include a new three-story addition on the north side of the Evergreen campus providing 40 new spacious and up-scale independent apartments attached to the main building. Also included in Phase 6 will be moving the Evergreen main entrance a little further north, creating additional parking, adding a new chapel and life enrichment area, plus a new library and more common spaces.
